Welcome to the reporting site of UofL Hospital, UofL James Graham Brown Cancer Center and UofL Physicians.
We are committed to an environment where open, honest communications are the expectation, not the exception. We want you to feel comfortable approaching your supervisor or management in instances where you believe violations of laws or policies may have occurred.
In situations where you prefer to place an anonymous report in confidence, you are encouraged to use the Compliance Hotline, hosted by a third-party provider, Compliance 360. To make a report you may use the toll-free telephone number listed at the right or submit a report via this online portal page. The information you provide through the Hotline will be sent to the organization by the third-party vendor on a confidential and anonymous basis. Please provide as much information and detail as possible so that the concern can be thoroughly addressed. All good faith reports, regardless of whether they are substantiated, may be made free from fear of retribution per our Non-retaliation/Speak Up Policy.
You are encouraged to submit reports relating to suspected violations of laws, regulations or policies. You also may simply ask for guidance related to policies or procedures. The Hotline is not a complaint line. Matters that do not involve violations of law or policy should be addressed through normal administrative channels.
If you are unsure of how to proceed, submit a report via this online portal page or use the toll-free telephone number listed at the right.
